In 84 people with amyotrophic lateral sclerosis (ALS), the Edinburgh Cognitive and Behavioural ALS Screen (ECAS) and Mini-Mental State Examination (MMSE) identified cognitive impairment at the same overall rate, 22.7%. However, the two tests agreed on impairment status in only 52.9% of cases. The findings suggest that the MMSE is less suitable for detecting early cognitive changes in ALS, while ECAS scores may provide more detail at lower levels of dysfunction.
Why this matters
This may help clinicians choose cognitive screening tools for people with ALS, particularly when early or milder impairment is suspected. The study does not show that either test improves treatment or outcomes, and it does not by itself change clinical practice.
Limitations and context
This was a single study of 84 ALS patients using German-language versions of the tests. It examined test performance and agreement, not treatment effects or patient outcomes. Although all participants completed the ECAS, 11% could not complete the MMSE. Further evidence would be needed to confirm how well these findings apply to other populations and clinical settings.
