A cross-sectional study found differences in gut microbiota between people with amyotrophic lateral sclerosis (ALS) who had a percutaneous endoscopic gastrostomy (PEG) tube and those who did not. People with PEG had lower levels of some short-chain fatty acid-producing bacteria, including Faecalibacterium and Lachnospira. The study also found associations between certain bacteria and nutrient intake.
Why this matters
The findings may help guide future research into how nutrition and feeding route relate to the gut microbiome in ALS. They do not show that PEG causes these changes, that the bacterial differences affect ALS progression, or that microbiome-targeted treatments benefit people with ALS.
Limitations and context
This was a cross-sectional study, so it cannot establish cause and effect. The sample was limited, and the authors describe the PEG-related findings as exploratory. Comparisons with the external control group may have been affected by differences in sample collection, DNA extraction and sequencing methods. The study provides a basis for future research, but treatment effects and clinical relevance still need to be demonstrated.
